- New Patient Intake form: your name, date of birth, contact details and address, emergency contact, your wellness goals, and your medical history, medications, supplements, allergies and family history, as requested on the form.
How the New Patient form is protected
Your intake form is encrypted in your browser before it is sent, travels in encrypted form, and is delivered only to The Longevity Society for review in preparation for your care. The company that hosts and supports this website works with us under a HIPAA Business Associate Agreement.
Please don’t send detailed health information by general email or social media messages, which aren’t designed for it. Use the New Patient form or call us.
Information collected automatically
